Joining our collective is easy! All you need to do is come in with the following:

• Valid doctors recommendation
• Valid ID (ie: CA State ID, Drivers License Or U.S. Passport)

Upon providing the above, you will be asked to complete a New Patient Intake Form.(This form only takes about 1 minute to complete)

In most cases doctors recommendations are validated and processed in less than an hour.
